腦中風危險因子之一般處理準則：高血壓

一、高血壓與中風或心血管疾病之相關性

血壓的高低與發生中風或其他心血管疾病之風險，為一連續性之關係。一般而言，平均血壓每增加10mmHg，中風之危險性即增加30% [1,2]。高血壓的定義是人訂出來的，為避免混淆，聯合國衛生組織/國際高血壓學會準則委員會（WHO/ISH Guideline Committee）與美國全國聯合委員會（Joint National Committee，JNC）一致建議，高血壓定義為血壓高於140/90mmHg，正常血壓為<130/85mmHg，正常偏高血壓為130-139/85-89mmHg，輕度高血壓是指血壓140-159/90-99 mmHg，中度高血壓指血壓為160-179/100-109mmHg，重度高血壓為血壓≧180/110mmHg [3,4]。
中風或其他心血管疾病之危險度，除依據血壓之輕、中、重度外，必須同時考慮是否存在相關血管危險因子、中風或心臟病等。危險程度之分類，如表一。依據Framingham資料，追蹤高血壓病人十年，屬輕度危險群者，未來十年發生重大心血管意外之危險小於15%，中度危險群者為15-20%，高度危險群為20-30%，而超高危險群之危險度則超過30%。因此，有高血壓而且併有危險因子者，已屬中度以上危險群，必須嚴格控制血壓，避免中風或其他心血管疾病之發生[5,6]。

二、高血壓之治療原則

治療高血壓病人的目標，為降低中風及冠心症之發生率及死亡率。治療高血壓時，也應同時治療或控制其他已知之危險因子，如吸菸、肥胖、高血脂或糖尿病等，如此，可使預防中風及冠心症之療效更為顯著。輕度高血壓者，若改變生活形態、戒煙、減輕體重、限制鹽分攝取、增加有氧運動等，可降低高血壓10mmHg左右[7-10]；至於中度或高度高血壓者，除改變生活形態外，必須使用抗高血壓藥物，若使用單一藥物仍未有效控制血壓時，可考慮同時使用多種抗血壓藥物，以便取得加成之降壓效果[11,12]。
治療高血壓之準則，美國全國聯合委員會（JNC）於2003年5月發表之建議重點為：(1)年齡大於50歲、收縮血壓高於140mmHg，是心血管疾病的重要危險因子。(2)年齡大於70歲，血壓從115/75 mmHg開始計算，每增加20/10mmHg，罹患心血管疾病的風險即加倍。(3)當收縮壓120-139mmHg或舒張壓80-90mmHg時，就應視為高血壓之前兆，需要改變生活習慣，以促進健康，預防心血管疾病。(4)大部分單純的高血壓病患，應先使用或併用Thiazide類利尿劑，而某些高危險病患，可從其他類抗血壓藥物開始投藥。(5)大部分高血壓病患，終需服用兩種以上藥物，以達到理想的控制目標；一般病患血壓控制到低於140/90mmHg，糖尿病及慢性腎病患血壓更應嚴格控制在小於130/80mmHg。(6)如果血壓比控制目標高出20/10mmHg，就必須考慮從合併兩種藥物開始投予治療，通常其中一種會選擇Thiazide類利尿劑[4]。治療高血壓之流程，如圖一所示。

三、高血壓藥物之選擇原則

目前常用高血壓藥物之藥理作用機轉為：(1)利尿劑Diuretics，主要作用為，因利尿作用增加血中鈉離子排出，及降低心臟之輸出量。(2)乙型交感神經接受體阻斷劑（Beta-blocker），主要作用在心臟之β腎上腺素受體，使心跳變慢及降低心臟之輸出量，同時也可作用於腎臟減少renin之釋出，和抑制中樞腎上腺素之分泌。Beta-blocker之優點為解除焦慮、減低心跳，缺點為不能用於氣喘病或房室傳導阻滯病人。(3)甲型交感神經接受體阻斷劑（Alpha-blocker），主要作用在α腎上腺素受體，而產生血管擴張，降低全身血管阻力，達到降壓效果。優點為可改善血脂異常和解除攝護腺肥大症狀，缺點為易造成姿態性低血壓，與鈣離子阻斷劑（calcium channel blocker）併用時，此副作用更為明顯，因此不宜用於aortic stenosis病患。(4)鈣離子阻斷劑（calcium channel blocker，CCB），主要作用於週邊血管，可降低週邊血管阻力，達到降壓效果，其次作用於腎臟，有利尿作用。此藥物之優點為適用於冠心症或糖尿病病人，缺點對心臟衰竭病人和心臟傳導阻礙（heart block）者不適用。(5)血管收縮素（Angiotensin-converting enzyme inhibitor，ACEI）或血管收縮素Ⅱ拮抗劑（Angiotensin receptor blocker，ARB），直接作用在週邊血管，降低週邊血管阻力，達到降低血壓之作用，其次作用在腎臟，有利尿作用。此藥為目前最安全有效之藥物，可有效降低心衰竭及腦中風之罹病率及死亡率，對糖尿病之腎臟病程也有抑制作用，但是此藥不適用在高血鉀症、懷孕和雙側腎動脈狹窄病人。
至於，各類常用降高血壓藥物之臨床試驗結果，可作為選擇藥物之依據，如表二。
對於中風病人之高血壓控制，JNC建議使用Diuretic及ACEI。使用Diuretic之原因為：(1) Diuretic為目前所有降血壓藥物中，最便宜而且有效的。(2)低劑量每天使用25-50mg hydrochlorothiazide或12.5-25mg chlorthalidone，產生痛風之機會很低，而且在TOMHS [24]報告中，產生性功能障礙之機率與控制組相較，並無差異性。(3)因使用利尿劑，而產生代謝異常副作用，利尿劑比CCB或ACEI稍高，但是追蹤使用利尿劑之糖尿病病人，與使用CCB或ACEI相較，並沒有增加心臟血管疾病之人數[19]。至於ACEI使用於初次中風預防之臨床試驗報告更多；HOPE臨床試驗，使用Ramipril 10 mg qd較placebo更有效降低中風之發生率[17]；但ALLHAT[19]試驗顯示，在初次中風預防上，使用Diuretic或CCB比ACEI多降低15％中風發生率，但此種差異未達統計學意義；至於，再次中風之預防，PROGRESS[13]指出，Diuretic（Indapamide）與ACEI（Perindopril）同時合併使用，能有效地降低中風發生率達43％。至於ARB對預防中風而言，LIFE [18]臨床試驗，使用ARB較β-blocker更有效降低中風之發生率。總而言之，對預防中風而言，適當控制血壓，比改善內皮細胞功能等理論，更為重要。因此，不論初次或再次預防，都應使用Diuretic為第一考量，並且劑量宜用低劑量25-50mg hydrochlorothiazide，若結果仍未達到理想降壓效果時，可考慮添加ACEI或CCB。

四、急性中風時高血壓之處理

急性中風時，血壓之增高，可提高腦血流量，改變腦組織之缺血狀態。美國中風學會建議，急性中風期間，收縮血壓在220mmHg或舒張血壓在120-140mmHg時，應使血壓慢慢下降10-15%。若舒張血壓超過140mmHg時，可使用靜脈點滴Sodium nitroprusside，並監控血壓，慢慢下降10-15% [25]。若使用抗凝血劑或血栓溶解劑tissue plasminogen activator（t-PA）治療時，血壓應控制在180/110mmHg範圍內，血壓在此範圍內，不會造成腦部二度傷害[26]。
急性中風時，70%病人的血壓會升高，此時之高血壓是中風的原因或結果，仍有爭議；但是，過高或過低的血壓，均是預後不良之指標[27,28]；如此，過高的血壓或過低的血壓，也許是需要治療的。小規模的臨床試驗ACCESS，使用Cardesartan來治療急性腦中風之病人，發現實驗組之預後比對照組好；不過，治療血壓的好處，是降低心臟病之發生，而不是中風[29]。急性中風時，過高的血壓應予治療，但是多高的血壓才需要治療，目前仍缺大型臨床試驗佐證。

五、結論
  1. 高血壓是中風多重危險因子中最重要的，而且是可以治療的，控制高血壓，可有效預防初次或再次中風。
  2. 若以預防中風為主要目的時，選擇降高血壓藥物，應先考慮Diuretics，另外，添加ACEI可達加成效果。若因為副作用之緣故，無法使用Diuretics或ACEI時，可考慮使用CCB或ARB。
  3. 急性中風時，降低血壓是很危險的，避免過度降低血壓，使用tPA時，必須把血壓控制在180/105mmHg左右，避免造成腦組織二度損傷。
  4. 長期高血壓之治療，血壓應控制在140/90mmHg之下，有糖尿病危險因子之病人，應更為嚴格，血壓宜控制在130/80mmHg；控制高血壓時，應注意病人之耐受度，特別是頸動脈狹窄嚴重之病患。
中風治療與處理共識小組：

　　為凝聚治療共識，並傳承臨床經驗，學會於民國91年1月26日舉行「中風治療與處理共識」會前會，確立共識之主題、研討會舉辦之原則與流程，並由胡漢華醫師任召集人，負責推動進行。
本準則於民國93年2月28日在台灣腦中風學會年會中舉行研討會，凝聚各界共識。於民國93年8月7日舉行小組會議，針對撰寫之準則內容進行討論。下列為本次共識小組及會前會成員（依姓名筆劃排列）。
